Abstract: THE INFORMATION COLLECTED BY THE USE OF FORM SSA-8740 IS NEEDED TO ENABLE SSA TO REFER RECIPIENTS TO AN APPROPRIATE STATE AGENCY FOR DRUG ADDICTION/ALCOHOLISM TREATMENT.
