Abstract: THIS FORM IS USED TO GATHER INFORMATION ON THE COST AND QUALITY OF FUELS DELIVERED TO ELECTRIC POWER PLANTS. THE RESPONSES ARE USED IN EVALUATION OF INDIVIDUAL UTILITY COSTS AND PRACTICES IN RATE CASES, AN IN THE REQUIRED PERIODIC REVIEWS TO INSURE EFFICIENT USE OF RESOURCES. THE DATA ARE ALSO USED TO CERTIFY SMALL POWER PRODUCTION FACILITIES AN COGENERATION PLANTS UNDER THE COMMISSION'S QUALIFYING FACILITIES
