THE INFORMATION WILL BE GATHERED FROM PRODUCERS WHO ARE SUBMITTING A REQUEST FOR ELIGIBILITY UNDER THES PROGRAMS. THE INFORMATION WILL BE USED TO DETERMINE PRODUCER ELIGIBILITY.
