Terms of Clearance: In accordance with the Paperwork Reduction Act and its implementing regulations, 5 CFR 1320, this information collection has been approved for use through December, 1992. Should the agency choose to resubmit this information collection for clearance at that time, its shall include an explanation of any changes contemplated by the Department- wide ("Easy Access") effort to reduce paperwork burden on program participants. Also included shall be justification of changes propose based upon improved information technology and a list of contacts with phone numbers of people the agency contacted while considering any changes.
