AUTHORIZED THE ISSUANCE OF THE CHECK. THE INFORMATION IS USED TO
DETERMINE IF THE CLAIMANT IS ENTITLED TO THE PROCEEDS OF THE CHECK.
THE LIKELY CLAIMANTS ARE INDIVIDUAL RECIPIENTS OF CHECKS.
