Abstract: THE FNS-66 AND FNS-67 ARE USED IN APPROVING SCHOOL AND CHILD CARE INSTITUTIONS WHICH ARE ELIGIBLE TO RECEIVE CASH REIMBURSEMENT AND/OR COMMODITY ASSISTANCE FOR FOOD SERVICE SERVED TO CHILDREN. THE FORMS A USED IN STATES WHERE THERE IS NO ENABLING STATE AGENCY TO ADMINISTER SCHOOL NUTRITION PROGRAMS AUTHORIZED UNDER THE NATIONAL SCHOOL LUNCH A AND THE CHILD NUTRITION ACT OF 1966.
