The FNS-66 and FNS-67 are used in
approving school and child care institutions which are eligible to
receive cash reimbursements and/or commodity assistance for food
served to children. The forms are used in States where there is no
enabling State agency to administer school nutrition programs
authorized under the National School Lunch Act and the Child
Nutrition Act of 1966.
