Terms of Clearance: This questionnaire is approved for use through August 31, 1994, to enable SBA to examine whether using a single form and sampling method- ology would improve the evaluation of its counseling and training pro- grams. (Currently, the agency uses four such forms approved under fou separate OMB control numbers.) Following this assessment, SBA should prepare a single clearance request covering the remaining form or form or seek a generic clearance under the recent OMB guidance for customer surveys.
