Abstract: THE ORGANIZATIONAL SURVEY IS A FORM WHICH REQUESTS BACKGROUND INFORMATION ON PROSPECTIVE GRANTEE ORGANIZATIONS. THE NEH WILL REQUIR A NONPROFIT ORGANIZATION TO SUBMIT A COMPLETED "ORGANIZATIONAL SURVEY" WHEN THE ORGANIZATION HAS NEVER BEFORE RECEIVED AN NEH GRANT AND/OR WHEN THE NEH STAFF HAVE QUESTIONS ABOUT THE ORGANIZATION'S VIABILITY OR FINANCIAL MANAGEMENT.
