this information collection, under the condition stated by NEH that
survey data do not constitute sufficient grounds for non-awardal of
a grant. NEH uses this survey to determine which grantees merit
examination of their organizational capacity. The survey is used
primarily for the media arts program, in which audits have
discovered that certain categories of grantees have posed problems
related to financial accountability.

THE ORGANIZATIONAL SURVEY IS A FORM
WHICH REQUESTS BACKGROUND INFORMATION ON PROSPECTIVE GRANTEE
ORGANIZATIONS. THE NEH WILL REQUIRE A NON-PROFIT ORGANIZATION TO
SUBMIT A COMPLETED "ORGANIZATIONAL SURVEY WHEN THE ORGANIZATION HAS
NEVER BEFORE RECEIVED AN NEH GRANT AND/OR WHEN THE NEH STAFF HAVE
QUESTIONS ABOUT THE ORGANIZATION'S VIABILITY OR FINANCIAL
MANAGEMENT.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
