Abstract: THE "ORGANIZATIONAL SURVEY" IS A FORM WHICH REQUESTS BACKGROUND INFORMATION ON PROSPECTIVE GRANTEE ORGANIZATIONS. THE NEH WILL REQUIRE A NON-PROFIT ORGANIZATION TO SUBMIT A COMPLETED "ORGANIZATIONAL SURVEY WHEN THE ORGANIZATION HAS NEVER BEFORE RECEIVED AN NEH GRANT AND/OR WHEN THE ENDOWMENT STAFF HAVE QUESTIONS ABOUT THE ORGANIZATION'S VIABILITY OR FINANCIAL MANAGEMENT CAPABILITIES.
