THE INFORMATION COLLECTED BY FORMS SSA-4305.1, 4305.2, AND 4305.3 WILL BE USED BY THE SOCIAL SECURITY ADMINISTRATION (SSA) TO EVALUATE AND TE THE EFFECTIVENESS OF THE 800 NUMBER SERVICE THAT IS CURRENTLY UTILIZED BY THE PUBLIC TO OBTAIN GENERAL INFORMATION AS WELL AS THE FILING OR SERVICING OF CLAIMS UNDER THE VARIOUS SOCIAL SECURITY PROGRAMS.
