instrument to incorporate the new disclosure statements pursuant to
the Paperwork Reduction Act of 1995. For the public docket, NCHS
must submit to OMB the revised survey instrument and disclosure
statement; and 2) the next submission for OMB review includes a
comparison of the NHHCS (in particular the current and discharged
patient questionnaires) with the home health section of MEPS, and
an analysis of further opportunities for coordinating between these
two survey efforts.

The National Home and Hospice Care
Survey will be conducted by personal interviews among a sample of
hospices and home health agencies about the patients they serve.
The survey will provide estimates of the characteristics of people
being served by hospices and home health agencies.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).

the proposed collection of information, that the certification
