Terms of Clearance: Approved; DOL submitted changes and additional information on 6/11/96 and 6/18/96. It is noted that this action is a reinstatement with change of an information collection. DOL stated in comments on 6/18/96 that DOL continued to collect this information even though approval had expired. DOL will actively track expiring collections to avoid a similar situation in the future. The request to display the expiration date and updated OMB number in an Employment Service Program Letter rather than on the form is approved. However, if the form is revised DOL shall revise the displayed OMB control number.
