The agreement States are requested to voluntarily participate in a monthly automated collection (which may include the NMED data screen and replaces NRC forms 565 and 566) of licensee data on the occurance of incidents and events involving the use of radioactive byproduct material, such as medical misadministrations, radiation overexposures, etc.
