This information
collection request (ICR) is approved through June, 2000. However,
the data collected through this instrument should not by itself be
considered sufficient to support regulatory action. In order to
support a rulemaking, the Commission should undertake a
better-controlled information collection effort.

The agreement States are requested to
voluntarily participate in a monthly automated collection (which
may include the NMED data screen and replaces NRC forms 565 and
566) of licensee data on the occurance of incidents and events
involving the use of radioactive byproduct material, such as
medical misadministrations, radiation overexposures, etc.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
