Abstract: Form is used for protecting HUD's multifamily housing programs by ensuring only responsible individuals and organizations participate. HUD will use this form to evaluate the feasibility of applicants with respect to their previous track records. Respondents such as owners, managers, consultants, general contractors, and nursing home operators and administrators will be subject to review.
