Terms of Clearance: Approved as amended by DOL's final revision of 12/10/97. This approval is for a limited time period because of the approach the agency will take in assessing qualifications of applicants. DOL must demonstrate the success of this approach in any future request for approval of this form. that would ask applicants to identify themselves using name and Social Security number. Additionally, the agency should consider collecting data on specific disabling conditions of applicants and address this in any future request to approve this form.

Abstract: This survey, to be completed voluntarily by job applicants, will provide information on the applicants' gender, race or ethnicity, disability, and the applicants' source of information on the job vacancy. This data will be used to evaluate the effectiveness of various recruitment methods employed by the Department of Labor and to tailor recruitment to meet equal employment opportunity objectives, by ensuring a diverse pool of applicants.
