Abstract: The Uniform Data System (UDS) is the Bureau of Primary Health Care's annual reporting system for grantees. The UDS includes reporting requirements for grantees of the following programs: Community Health Center, Migrant Health Center, Health Care for the Homeless, Outreach and Primary Health Services for Homeless Children, and Health Services for Residents of Public Housing.
