The purpose of these forms is to
collect surveillance data on antimicrobial resistance in Neisseria
gonorrhea. These data are used by public health officials at the
national, State, and local level to determine appropriate therapy
for gonorrhea. In addition, the data are used to monitor trends in
certain epidemiologic aspects of gonorrhea, including the Healthy
People 2000 goal for repeat gonococcal infections.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
