In these circumstances, the AoA should submit a note for the OMB public file and a new burden estimate via a correction worksheet._In addition, the AoA must incorporate the disclosure statements mandated by the Paperwork Reduction Act of 1995. For the public record, the AoA must submit to OMB the revised forms/instructions.
