information on Diabetes Control Programs on a quarterly basis
without OMB approval. This is a violation of the Paperwork
Reduction Act and the Agency will be cited for the violation in the
next ICB. CDC should ensure that such violations of the PRA do not
occur in the future.

A Management Information System is
being built by the Division of Diabetes Translation, Centers for
Disease Control and Prevention, to comprehensively collect
information associated with activities involving the grantees of a
national chronic diabetes program. The web-based system employs a
uniform systematic method of collecting information concerning
activities of programs funded by CDC.
On behalf of this Federal agency, I certify that
