CDC provides funding to diabetes prevention and control programs (DPCPs) in 50 states, the District of Columbia, Puerto Rico, and the Virgin Islands. CDC collects program and progress information from funded programs to monitor performance and support program needs for technical assistance. CDC is transitioning to an enhanced MIS for diabetes programs and a common set of performance indicators that will be harmonized with the information collections planned for other chronic disease prevention and control programs.
