Abstract: This form accomplishes the Statutory mandate for data collection in Title VII of Rehabilitation Act of 1973, as amended (Act) and implementing regulations. These data collection requirements are found in sections 13, 706, 721, and 725 of the Act and 34 CFR Part 366.
