changes and revised forms consistent with ACF memos of 07/17/02 and
07/18/02. The revisions noted in ACF memos will be transferred to
the forms included in ACF's web-site. If Congress reauthorizes
CAPTA, HHS will submit to OMB any revisions made to the information
collection requirements. Upon subsequent submissions, HHS will
submit to OMB the Program Instruction (PI) that is issued to
respondents and that reiterates the regulatory and statutory
requirements for the information collection.

Under title IV-B, subparts 1 and 2, of
the Social Security Act States and Indian Tribes are to submit to
the Department a five year Child and Family Services Plan (CFSP),
an annual progress and services report, and an annual budget
request and estimated expenditure report (CFS-101). The plan is
used by States and Indian Tribes to develop and implement services
and describe coordination efforts with other federal, state and
local programs. The Annual Progress and Services Report is used to
provide updates and changes in the goals and services under the
five year plan, as well report on activities conducted
