CDC has been collecting this information without an OMB number for the past 6 years. This is a serious violation of the Paperwork Reduction Act and must be reported in this year's Information Collection Budget. In the future, CDC should take great care to ensure that all collections of information from the public are identified and have valid OMB control numbers.
