Abstract: OMB clearance is being sought to collect information for a series of surveys to monitor and evaluate the pilot site launch of the National Kidney Disease Education Program. NIDDK will conduct a survey to monitor and evaluate the effects of a pilot kidney disease education program. This will be accomplished through baseline and follow-up surveys of the primary target audience members, i.e. African American adults and health care provides, in four pilot site locations (e.g., Atlanta, GA, Baltimore, MD, Cleveland, OH, and Jackson, MS).
