collect information for a series of surveys to monitor and evaluate
the pilot site launch of the National Kidney Disease Education
Program. NIDDK will conduct a survey to monitor and evaluate the
effects of a pilot kidney disease education program. This will be
accomplished through baseline and follow-up surveys of the primary
target audience members, i.e. African American adults and health
care provides, in four pilot site locations (e.g., Atlanta, GA,
Baltimore, MD, Cleveland, OH, and Jackson, MS). The research is
