This information
collection request is approved through 12/31/04 consistent with the
following terms of clearance: (1) Given the that the delay in
initiating the media campaign will also delay the completion of
data collection, CMS will conduct analyses to distinguish between
the effects of the media campaign and other possible causes of
differences in the 2 data collection periods. (2) In reporting the
results of this survey, CMS will carefully qualify any results,
explaining the potential impact of any delay in completing data
collection.

CMS - CBS needs to collect these data
to pilot test a national campaign to educate current and future
Medicare beneficiaries and their families about long term health
care needs, as requested in the Presidential Initiative for Fiscal
Year 2000 Budget. Project findings will be used to design and
implement a nationwide campaign. Respondents will be from two
groups: 55-70 year-olds and persons with disability who are 18-64
years of age.
On behalf of this Federal agency, I certify that
