Abstract: DOT agencies require a number of employers to submit end of year reports concerning their drug and alcohol testing programs; 21 different forms are currently in use. OST will reduce that to a single one-page form for use by DOT agencies. This PRA submission for the MIS forms have previously been included in DOT agency PRA submissions.
