Terms of Clearance: CDC will resubmit a revised version of the collection, which will be conducted by CDC partner organizations, for OMB review prior to fielding the survey instrument. CDC is reminded that 5 CFR 1320.3 indicates "A Federal agency is considered to conduct or sponsor" a collection of information if the agency collects the information, causes another agency to collect the information contracts or enters into cooperative agreement with a person to collect the information...or in similar ways causes...a partner in a cooperative agreement...to obtain, solicit, or require the disclosure ...of information by or for an agency."
