The clearance of
this information collection request is approved, with terms of
clearance binding the FCC to actions agreed upon by OMB, FCC, and
SBA - regararding burden reduction measures associated with the
Forms 499A and 499Q. Under this agreement, VOIP providers will be
required to submit limited information, and not the entire extent
of Forms 499A and 499Q. Specifically, VOIP providers will be
required to submit information in Blocks 1, 2, and 6 of Form 499 A,
and Blocks 1,2,4, and lines 114, 119, and 120 of Form 499Q. In the
short term, VOIP providers contributions to the USF will be based
upon their projected gross interstate and international
telecommunications revenues, as described in a forthcoming Public
Notice from the FCC. The period of clearance will be for 4 months,
expiring on November 26, 2006. As a terms of clearance, by November
1, 2006, FCC will be required to meet with OMB to discuss the
requirements entailed in the traffic study calculation
methodologies.

The Federal Communications Commission
(Commission) requires all telecommunications providers and certain
other providers of telecommunications, such as providers of
interconnected voice over internet protocol (VoIP) services to
contributed to universal service. Wireless and interconnected VoIP
providers are required to contribute based either on their actual
interstate end-user gross-billed telecommunications revenues,
interim safe harbors, or traffic studies. The wireless safe harbor
