Approved consistent with CDC memo submitted to OMB clarifying that CDC will remove the willingness to pay (WTP) sub-study from this clearance request and revise the WTP study to address OMB questions and comments. If CDC resubmits a separate revised package for the WTP study OMB will waive the 60 day comment period for resubmission. Regarding non-response bias analysis, CDC shall report to OMB on the results of its non-response analysis upon conclusion of this clearance period.

The magnitude of the injury problem is such that a dedicated means of collecting national data is needed. The data collected from this survey will allow CDC to measure the prevalence of risk factors for injury, define populations most at risk, allow the tracking and monitoring of several Healthy People 2010 Objectives, and monitor the impact of interventions for injury prevention and control.

The ICARIS-2 Phase-2 (0920-0513) OMB package was initially submitted in September of 2003. The submission was suspended in January of 2004 due to problems with the interviewing scheduler process for the ICARIS-2 (Phase-1) survey. The problem was discovered some time after completion of the Phase-1 component of the survey in February of 2003. The suspension was based on the rationale that Phase-2 could not begin until problems with the administration of the initial ICARIS-2 survey had been resolved. At the time of suspension, the OMB package for ICARIS-2 Phase-2 was close to being cleared. The project is resubmitted now in order to continue as intended before. However, because the previous iteration of the project is no longer active, there is no current OMB inventory for this project, making the proposed burden of 620 hours an increase from the present 0 hours.
