When resubmitting to OMB, please 1) certify the collection, 2) clarify estimates in section A12 and verify that they are consistent with the burden estimates provided on the information collection form (1 hour) and the ROCIS worksheet, 3) clarify cost to the government in section A14 and verify that this amount is consistent with the amount reported on the ROCIS worksheet, and 4) clearly indicate in section A-17 whether FAA is requesting approval not to display the expiration date for OMB approval; the supporting statement suggests approval not to display yet the form currently displays the expiration date.

The administrator has determined, based on evaluation of previous accidents and other incidents, that certain events involving malfunctions and defects may be precursors to t he recurrence of these accidents. As a result, operators and repair stations are required to report any malfunctions and defects to the Administrator.
