The CDC seeks to improve the oral
health of the nation by targeting efforts to improve the
infrastructure of state and territorial oral health departments;
strengthen and enhance program capacity related to monitoring the
population's oral health status and behaviors; develop effective
programs to improve the oral health of children and adults;
evaluate program accomplishments; and inform key stakeholders,
including policy makers, of program results. CDC requests the
submission of semi-annual status reports from each funded program.
The proposed information collection will be used to identify
training and technical assistance needs; monitor compliance with
cooperative agreement requirements; evaluate the progress made in
achieving national and program-specific goals; and respond to
inquiries regarding program activities and effectiveness.
The CDC Division of Oral Health
(DOH) proposes a change in data collection methodology by the
development of an automated management information system (MIS) to
maintain individual information and to normalize the information
reported by programs supported by CDC DOH Cooperative Agreement #
3022.
