Approval from Oklahoma IRB is contingent upon changes, i.e., modification of the consent and
inclusion of assent, and upon receipt of a Certificate of Confidentiality.
The requested modifications have been made, and the application for Certificate of
Confidentiality is in progress. An amendment to the Oklahoma IRB will be submitted when the
Certificate of Confidentiality is received.
