(3) In column three, provide the total number and percentage of all students identified as LEP by each State-selected ELP assessment(s) (“identified” refers to the number of students determined to be LEP on State-selected ELP assessments).

(4-7) In columns four-seven, provide the total number and percentage of all students identified as LEP at each level of English language proficiency as defined by State-selected ELP assessment(s). The number (#) and percentage (%) of columns 4-7 should equate to the number (#) and percentage (%) of all students identified as limited English proficient in column 3.

(3-7) In columns three-seven, provide the total number and percentage of LEP students at each level of English language proficiency who received Title III services during the 2004-2005 school year. The number (#) and percentage (%) of columns 3-7 should equate to the number (#) and percentage (%) of all students identified as limited English proficient in column 2.


(8) In column eight, provide the total number and percentage of LEP students who participated in a Title III language instruction educational program during the 2005-2006 school year and who were transitioned into a classroom not tailored for LEP children and are no longer receiving services under Title III.
