The U.S. Nuclear Regulatory Agency (NRC)has requested permission to use unchanged the Department of Labor’s OMB-approved Applicant Background Questionnaire for the same purposes as stated in the attached Supporting Statements (see supplementary documents).

Estimated NRC burden is as follows:

• 2,000 responses / respondents x 3 minutes (.05 hours) average response time = 100 hours.

The burden inventory for this OMB control number is updated to include new level of burden associated with NRC’s use of the form.
