This statement supports a request to obtain approval for a reinstatement of a previously approved information collection to conduct the school-based National Youth Risk Behavior Survey (YRBS). YRBS is a biennial survey of high school students that assesses priority health risk behaviors related to the major preventable causes of mortality, morbidity, and social problems in the U.S.

The minor changes incorporated into this reinstatement request include a slightly modified questionnaire and an updated title to accurately reflect the years in which the survey will be conducted. The 2009 and 2011 YRBS will use the previously OMB-approved sampling strategy, recruitment methods, and data collection procedures involving nationally representative samples of students in public and private schools, enrolled in grades 9-12.
