Annual Progress Reporting Form



Change in Language – any use of personal pronouns found in questions, requests for data, or instructions were removed. All parentheses () and brackets [] were removed. All italicized sentences were changed to non-italicized sentences.


The following Sections were deleted for the following reasons:

General Reporting Instructions – This section was deleted from the form for two reasons: 1) the instructions for each field are included and imbedded in each section of the form; and 2) RSA staff sends to all the grantees a letter reiterating the general reporting instructions no less than 45 days prior to the end of the FY, or, for grantees in their first year, 45 days before the six-month report is due.

Section 1 Reporting Period – This section was deleted because the reporting period will be displayed automatically in the header of each section of the form.

Section 7 Special Efforts, Section 13 Success Story(ies), and Section 14 Comments – All grantees are required to complete the ED 524B Form (coversheet only), which includes an Executive Summary section. These sections of the form were deleted and grantees will be encouraged to place data relating to these sections in the Executive Summary of the ED 524B Form.

Section 3 Project Goals – Section 3, which requested the number goals, was deleted, section 4 was changed to Section 2, and the title of Section 2 was changed from “Project Objectives and Goal Status” to “Project Goals and Objectives.


The elimination of these sections reduced the number of sections from 15 sections to 10 sections. These 10 sections are now: 1) General Information Section, 2) Section 1 – Budget Narrative, 3) Section 2 – Project Goals and Objectives, 4) Section 3 – Serving American Indians with Disabilities, 5) Section 4 – VR Services, 6) Section 5 – Educational Goals and Employment Outcomes, 7) Section 6 – Common Measures, 8) Section 7 – Interaction with State Agency, 9) Section 8 – Evaluation, and 10) Section 9 – Consumer Satisfaction.


The following sections and questions have auto-calculated fields – Section 1, question 1c; Section 3, question 3; and Section 5, Part B, question 3.


The new Section 2 Project Goals and Objectives: rather than having several Goal and Objective charts available, a button will be placed at the bottom of the first Goal and Objectives chart that says, “Add another Goal.”


The new Section 3 Serving American Indians with Disabilities: question 1, including the cumulative chart, was deleted since the answer to question 1 is found in questions 2 and 3. Questions 2 – 5 now become questions 1 – 4.


The new Section 3 Serving American Indians with Disabilities: question 1 – the sentence “this figure should match the figure reported in Chart 1 column 1 for the current grant year” was deleted.

Data in this section address the Government Performance and Results Act (GPRA) program indicators for the American Indian Vocational Rehabilitation Services Program and common measures established by the Office of Management and Budget (OMB) for all federal Employment and Training Programs.

Data reported in this section will be used to provide supplemental information for reporting on the Job Training Common Measures.


The new Section 6 Common Measures: the definition of “Employment Outcomes:” is deleted since it is already defined in the new Section 5. The definitions of “Earnings:” and “Exiting The AIVRS Program:” remain.


The new Section 6 Common Measures: questions 1 and 2 are deleted since the data for those questions are reported in the new sections 3 and 5. Questions 3, 4, 5, 5a, and 5b become questions 1, 2, 3, 3a, and 3b.
