SURVEY OF READING FIRST LEA SUBGRANT FUNDS

Purpose of this survey: The Reading First Program (RF), authorized by the Elementary and Secondary Education Act (ESEA) of 1965, as amended by the No Child Left Behind Act of 2001 (NCLB), provides funds to State educational agencies (SEAs) to help states and local educational agencies (LEAs) use scientifically based reading research to improve reading instruction from kindergarten through third grade to achieve the goal of all children reading at grade level by the third grade. The purpose of this survey is to learn how LEAs use these funds.

Please complete the following survey and submit it following the instructions below by ________, 2009.

Instructions:

This survey is being conducted by RMC Research Corporation on behalf of the U.S. Department of Education. All responses will remain confidential, i.e., only RMC staff will see your individual responses. You must complete the survey online.



Go to the website address that was given to you in this e-mail, and enter the unique username and password that were sent to you. Complete the questionnaire online and submit it electronically.

PLEASE NOTE:

  • You may open, edit, and close the questionnaire as often as you like, and move anywhere within the questionnaire, without losing data before ____ PM, ______, 2009.

  • Data will be saved automatically to a database when you move from page to page, but you may also save your data without moving to another page by clicking the “SAVE” button.

  • If your questionnaire website is left open without any activity for longer than 8 hours (e.g., if you forget to close the questionnaire before you leave at the end of the day), you will “time out,” lose your connection with the server, lose your data, and have to start over again.
