Terms of Clearance: Prior terms of clearance remain in effect: This form is approved for government-wide use. Any grant-making agency that intends to use the survey under ED's approval must contact ED and provide the necessary burden information. ED will then submit a change worksheet (83-C) to OMB to account for the additional burden. ED will follow-up annually with all participating agencies to ensure that the burden hour calculation remains correct. Any changes to burden hour calculation will be adjusted through a 83-C. At this point in time, ED and grants.gov have identified this form as a potential candidate for adoption as a government-wide form under grants.gov. ED will continue to work with grants.gov to determine if grants.gov can assume burden and OMB clearance responsibilities for this form.

Abstract: To ensure equal opportunity for all applicants including small community-based and faith-based and religious groups, it is essential to collect information that allows Federal agencies to determine the level of participation of such organizations in Federal grant programs while ensuring that such information is not used in grant-making decisions.

This increase in burden hours is due to more community-based and faith-based agencies applying for federal grants and submitting the EEO Survey form for a variety of discretionary grant programs in the nine government agencies using the form.
