of blueberries is being asked to supply the
following data. Response is voluntary and not
required by law. However, cooperation is very
important in order to estimate accurately
blueberry production and value. Your individual
report is confidential and used only in
combination with other reports for statistical
purposes. Please promptly return this report in
the enclosed postage paid envelope, or fax it to
