Narrative of Revisions

The purpose of the Narrative of Revisions is to clearly indicate revisions to a collection since the previous approval

Changes to All Forms:

1. Added the burden statement, OMB form number, and form number expiration to the bottom of all pages.

2. User is required to register and login prior to submitting a capability gap or information request.

3. Moved the login button from the page header to the menu bar.

4. Moved the Funded Projects and National Threat Advisory web parts to the left hand pane of the screen.

DHS Form 10011: New Capability Gap Data Form

1. Changed the title of the form to Submit a Capability Gap.

2. Added eligibility instructions at the top of the form.

3. Added a spell check feature.

4. Changed the Disciplines field from a checkbox options to drop down options.

5. Removed the Employer field from the form.

6. Removed the Title field for file uploads.

7. Added a list of valid file types the user may upload.

8. Added character limit and count for the first three text fields.

DHS Form 10012: Information Request Form

1. Changed the title of the form to Information Request.

2. Added instructions, contact phone number, and contract email at the top of the form.

3. Added spell check.

4. Added character limit and count for the text field.

DHS Form 10015: User Registration Form

1. Changed the title of the form to Register.

2. Removed the instructions at the top of the form.

3. Added field “Confirm Email Address”.

4. Added field “Ext.” to the Phone number field.

5. Removed field “First Responder Discipline”.

6. Added field “Employer”.

7. Removed “Comments” field.

8. Added checkbox for the user to acknowledge they have read and agree to the Rules of Behavior.
