Your answers to the questions in this report are needed to prepare production and value estimates for the 2008-2009 macadamia nut season. All questions concerning production and purchases refer to the period beginning July 1, 2008 and ending June 30, 2009. Please complete this questionnaire and return it by June 9, 2009. A postage-paid envelope is enclosed for your convenience or you may fax the report to 1-808-973-2909.
Response to this survey is voluntary and not required by law. However, your cooperation is extremely important in order to make accurate macadamia nut estimates for Hawaii.
