This Revision ICR supports continued collection of progress and activity information from health departments in selected states and the District of Columbia. Respondents are funded through the National Heart Disease and Stroke Prevention Program. Information collected through the electronic MIS is used to monitor and evaluate programs, and to provide technical assistance to NHDSPP awardees.

Although there will be an increase in the number of awardees funded through the NHDSPP, reporting requirements involving the MIS will be discontinued for awardees funded through the WISEWOMAN program. No changes are proposed to the information collection instrument, the burden per response, or the frequency of information collection.
