This revision request is to support semi-annual collection of progress report information from recipients of Heart Disease and Stroke Prevention Program funding, and WISEWOMAN funding. Programs are funded by CDC's Division for Heart Disease and Stroke Prevention to design, implement, and evaluate public health prevention and control strategies to reduce disease, disability and death related to heart disease, stroke, and other behavior risk factors for chronic diseases. Information is collected via an electronic Management Information System.
