The Centers for Disease Control and Prevention (CDC) seeks review and approval of an automated Management Information System (MIS) to maintain individual state heart disease and stroke prevention information, and to provide a consistent format for the information reported by these programs. The proposed web-based MIS will employ a more formal, systematic method of collecting progress reports and continuation applications.
