Information is collected under
authority of Parts 608 and 609, of the Higher Education Act of
1965, as amended (HEA). The information collection of this
discretionary grant application package falls under the
Streamlining Plan OMB Number 1894-0001. The collection of
information is gathered in two phases: Phase I - Formula Data and
Phase II - The Project Plan. The collection of information is for
the purpose of obtaining institutional and budgetary information
needed to evaluate applications. Phase I will continue to be
submitted by way of facsimile. Both phases are submitted
individually by a select number of eligible institutions of higher
education. Both phases have separate closing dates. The two-page
Phase I Formula Data that is collected is used to determine the
amount of individual grants based on a prescribed formula.
The Higher Education
Opportunity Act of 2008 (P.L. 110-315) increased the number of
Historically Black Graduate Institutions (HBGI) or Qualified
Graduate Programs (QGP) by six applicants; therefore a program
change burden hour increase of 96 has occurred, i.e. 16 average
hours times six additional applicants.
