The Health Hazard Evaluation (HHE) Program of the National Institute for Occupational Safety and Health (NIOSH) is providing the attached brochure to inform you about our services. We are interested in opportunities where our program and yours can work collaboratively to address concerns about health hazards in workplaces in your area.

Please take a few minutes to review the brochure, then share your feedback with us. We invite you to complete an on-line survey. It should take about 10 minutes of your time.

This data collection is designed to help us assess the usefulness of the brochure so that we can enhance our relationships with local health departments. Your participation is critical to the success of this data collection.
