Abstract: SSA uses Form SSA-L4201 to collect information from employers to establish and verify wage information for Supplemental Security Income(SSI) claimants and recipients. SSA also uses the information to determine eligibility and proper payment for SSI. The respondents are employers of applicants for and recipients of SSI payments.
